I had to:
yum install postgresql91-contrib
/bin/su - postgres
psql <database>
CREATE EXTENSION fuzzystrmatch;

And that seemed to work.

apt-get install postgresql-contrib-9.0
/bin/su - postgres
psql database
\i /usr/share/postgresql/9.0/contrib/fuzzystrmatch.sql
